Daily Incident, Accident & Near-Miss Report

Purpose: 
To record, investigate and monitor any incident, accident, injury, near miss, damage or safety concern involving
customers, crew, contractors or other persons during Lagoon Mavericks operations.

All incidents should be reported, even where no injury occurs. If in doubt, report it.

Key Rule

If something happens that could reasonably have caused injury, damage or disruption — record it.

This includes near misses, even when nobody is injured.

Examples include:

  • Customer trips while boarding but is uninjured.
  • Customer slips on a wet deck.
  • Customer falls against a seat or fitting.
  • Customer falls overboard.
  • Customer falls while entering or exiting the water.
  • Water-skier falls awkwardly and reports pain.
  • Wakeboarder lands incorrectly and sustains an injury.
  • Knee boarder falls and impacts the water heavily.
  • Tubing passenger falls from the tube.
  • Customer collides with another participant.
  • Tow rope or water sports equipment behaves unexpectedly.
  • Life jacket becomes loose or requires adjustment.
  • Crew member slips while handling equipment.
  • Crew member strains themselves lifting or moving equipment.
  • Equipment breaks or becomes damaged.
  • A potentially dangerous situation occurs, but nobody is hurt.


The objective is not to assign blame. The objective is to record what happened, protect our customers and workforce, identify trends and prevent the same incident from happening again.
